The MOOD docu-series (w.t.) is a ground-breaking coming-of-age TV docu-series that places four American teenagers at the center of their own stories.
We’re casting nationwide to find four teens from various backgrounds who are on a journey of self discovery. We’re excited to hear your story.

The MOOD Docu-Series (w.t.) is in partnership with Mood.org, a non-profit online resource created to help teens cope with everyday stress and anxiety by offering a vast collection of tools they can use for free to normalize and neutralize their emotions. Executive producers for the docu-series include Mood Co-founder, Jane Hoffman, Rafael Monserrate, award-winning director and producer of Alone on History Channel, and Julie Monserrate, a veteran brand strategist, creative consultant and former Executive Creative Director at CBS Television (Survivor). They will be joined by Emma Choate (Love on the Spectrum) as Supervising Producer and Rebecca Rosichan of Extreme Makeover: Home Edition as Head of Casting

CASTING RELEASE

I warrant and represent that everything stated in this application is true, and that I have the full right and
authority to submit this application and grant all rights granted herein, and that I own or control all materials
I may submit along with this application. I understand that any falsely submitted answers can and will be
grounds for removal from the application process and from my subsequent participation in the final show or
series. I further acknowledge and accept that this application form and any other materials (including, but not
limited to, photographs, and videotapes) I have submitted or will submit to Butler Avenue, Inc., PSFW LLC, or
RR Casting, Inc., including any of their parents, affiliates, subsidiaries, licensees, and assigns, (hereafter
collectively referred to as “Producer(s)”) in connection with my application to participate in any show
Producer is, or will be associated with in the future (the “Program”), and any videotaping done or information
and materials collected by the Producers will become property of Producers, and will not be returned to me.
I hereby confirm that I (a) am at least 18 years of age or if I am under 18 have the express permission of my
parents and/or legal guardians as may be required by law (certified below); and (b) am a citizen of the U.S.
and able to legally travel to and re-enter the U.S. from any foreign country in connection with my
participation (if any) in the Program, OR (c) have U.S. Immigration status or a visa that allows me to stay
legally in the U.S. to participate in the Program, as determined by Producer in its sole discretion, and am able
to legally travel to and re-enter the U.S. from any foreign country in connection with my participation (if any)
in the Program. I am not a candidate for public office and will not become a candidate until one (1) year after
the initial broadcast of any episode(s) of the Program in which I appear.
I have disclosed to Producer, to the best of my knowledge, anyone I know, including any member of my
immediate family or anyone living in my household, who is now or has been in the past two (2) years, an
officer, director, employee, agent or representative of Producer, any television network that may air the
Program, any person or entity involved in the development, production or distribution of the Program, any
sponsor of the Program or its advertising agency, or any person or entity supplying any services to the
Program, including prizes, if applicable. I understand that Producer reserves the right to render ineligible
anyone whom Producer determines is sufficiently connected with the production, administration, judging, or
distribution of the Program such that his or her participation in the Program could create the appearance of
impropriety. Conversely, Producer may allow any person to remain part of the Program who may be
acquainted with a person or entity connected with the production and/or administration of the Program if
Producer determines that such person’s involvement in the Program does not and will not affect the integrity
of the Program.
By submitting this application, I hereby consent to the recording, use and reuse by Producers of my voice,
actions, likeness, name, appearance, photographs, biographical material including any submitted social media
presence, and any information contained in, derived from or obtained in connection with my application to be
a participant in the Program or in any materials submitted by me in connection with my application
(collectively “Likeness”), including as edited, altered, or modified by Producer and any of its parents, affiliates,
subsidiaries or related entities, in any and all media now known or hereafter devised, worldwide in
perpetuity, in or in connection with the Program or any other work by Producers. Any offer to become a
participant shall be conditioned upon entering a long-form participant release with Producer and may be
conditioned upon submission to a background check in accordance with applicable laws, including medical
and psychological evaluations, and that I may be required to meet additional eligibility requirements and
terms and conditions.
I understand and acknowledge that being chosen to interview for the Program will not ensure that I will be
selected to continue through the selection process. Producer reserves the right to exclude, at their sole
discretion, any individual from the interview and selection process at any time. Producer also reserves the
right the change the rules and procedures at any time. I understand that by submitting my application to the
Program, I am agreeing to be exclusive to Producer until such time that Producer removes me from the
application process, and that I agree not to apply to any other programming during the application process.
I agree that Producer may license, assign, and otherwise transfer this application and all rights granted by me
under this application to any person or entity.

CONFIDENTIALITY. Whether or not I am selected to be a participant on the Program, I shall keep in strictest
confidence and shall not, and shall not authorize or cause others to, directly or indirectly, use or disclose to
any third party at any time (i.e., prior to, during, or after the taping or exhibition of any episode of the
Program) any of the following, which is and shall remain the exclusive property of Producer: any information
or materials that I may read, hear or otherwise acquire or learn in connection with or as a result of my
participation in or in connection with the Program, including without limitation, any information or materials
concerning or relating to Producer, the business of Producer, any program produced by Producer, including,
without limitation, any information concerning or relating to the Program, the Program applicants, the
Program participants, the location(s) of the Program, the events contained in the Program, the outcome of
any episode of the Program, and the contents of this application or any other agreement or document that I
sign, have signed or receive from Producer at any time (collectively, the “Confidential Information”).
I hereby release and indemnify Producer, the applicable network, and all of their respective licensees, assigns,
successors, parents, affiliated and subsidiary companies, divisions, sponsors, advertisers and joint ventures as
well as their employees, officers, directors and agents (collectively, the “Released Parties”) from any and all
claims or demands of every kind that I or any third party may now or hereafter have against the Released
Parties in connection with my application to or appearance in the Program, including all the materials I have
submitted in connection therewith, or any exercise by any or all of the Released Parties of any of the rights I
am granting hereunder, or any other matter contained herein, including, but not limited to, any claims for
defamation, violation of rights of privacy and/or publicity, negligence, and/or intentional infliction of
emotional distress.
In connection with the foregoing releases, I hereby expressly waive any and all rights and benefits conferred
by the provisions of Section 1542 of the California Civil Code or by any similar law or provision of any
jurisdiction throughout the world.
I hereby agree that my remedies for any cause of action, dispute or claim arising out of or in connection with
this application by Producer or others will be limited to an action for damages and in no event will I be
entitled to rescind any documents or materials related to this application, the Program, or seek injunctive or
any other equitable relief.
Any signed copy of this application, including electronic “e-signatures,” shall for all purposes be treated as if it
had been delivered containing my original signature, and shall be binding upon me in the same manner as
though an original signed copy had been delivered.
